I (we) grant the American Chinese School of Greater Detroit (ACSGD) the power to authorize medical treatment or medical procedures, at my (our) sole cost and expense, in case of an emergency situation to aid me (us) and/or my (our) child(ren) in connection with all activities organized by the ACSGD, at its selected location, such as Troy Community Center, Derby Middle School, Troy High School and International Academy. In further consideration of the services and instruction provided, I (we) hereby release the ACSGD and the facility owner, their directors, officers, teachers and volunteers from any and all claims or liabilities which may result from participation in the activities organized by the ACSGD at its selected facility, including but not limited to those arising as a result of negligence. I (we) have read carefully all the school policies and rules. I (we) agree to adhere to and accept all the school policies and rules, and understand they are subject to changes.
